It takes the core elements of the books, there have been some changes with the timelines and certain characters. Haven't read the 3rd book yet, but I still very much enjoy the show even w/some of the changes.

GOT isn't completely faithful either, as many things are different. Mostly out of necessity it seems.

Syfy is showing the uncensored version of the first season on Syfy On Demand and the Syfy app.
Syfy will have them available till December so I think I am going to go back and watch the entire season to see if being uncensored makes it demonstrably better.
