This is a failure of the pentagon, the defense department, and every department related to the military.

Each of those entities answers to a civilian up the chain, none of them are independent. Also, it appears as if no one who's discussing this is taking into account that per the previous administrations agreement with the Taliban, the withdrawal was supposed to happen in May. The Biden administration rushed the withdrawal to avoid the Taliban getting irritated and starting to attack US forces again.
