Probably common sense but most healthcare workers are cheating or in relationships with the people they work with. A lot of the time they are distracted by these relationships and rumors. I wouldn't be surprised if some standards of care are compromised because the doctor is pissed at his mistress nurse or two aids are arguing behind closed doors.
If you demand great care or anything above the minimal you'll probably piss off some nurse and they wil label you difficult and may treat you differently.
It doesn't matter how experienced you are, I'm willing to bet that 9 times out of 10 if you did not get a job in a healthcare facility it is because you are not related to or don't know someone who already works there.
Lots of places are not cleaned properly- medical places included. To save time, Some people don't use biohazard or bleach to clean blood spills or bathrooms- which is unsanitary to me because it's one of the few chemicals that kills HIV and hep c. They take about five mins to clean a bathroom, including barely wiping down a toilet and shower/bath. The solution is watered down and they don't allow it to sit long enough to let it kill all pathogens.
Many doctors listen to music at the operating table (most tv shows make it seem like the OR is quiet). Younger surgeons are singing along to death metal while operating on you.
