I think you just lose the normal medical fee for dying
I think the point is more of running terrified for your virtual life as friends spend their not-hard-earned virtual money on making your gameplay experience unpleasant. Quite humiliating. I however have grown fond of these muggers you can hire to go after players. Friends wont even know who are sending these sketchy individuals to rob them, it's extremely demonic.

I wish they had an option where you could just join games where people had bounties on their heads so i could collect my dough.
