In the dream, Rachel says he missed his flight - could be alluding the fact that 'dikk Whitman' is already dead in Don's mind, but also possibly that he could end up physically checking out.

I think she's alluding to the fact that he missed his chance with her. Remember he wanted the two of them to runaway to LA and start a life together? He missed that flight, while also missing his chance to be with her
