Because their residents are more sexually responsible, regardless of orientation.

As you stated, people who engage in anal sex contract it at a higher rate. Most women are not exclusively engaging in anal sex. I’d venture to say most aren’t sharing needles either. They may, however, be engaging in unprotected vaginal and possibly anal sex with men who are not exclusively heterosexual.
