Qui Gon failed by dying before he could do anything. Obi Wan was young not ready for a pupil. Annakin was always more a younger brother to him than a student. But Annakin had him, an entire Jedi council and some of the universes best diplomats at his side for most of his life and he STILL chose dark side. It’s still on him

The Jedi wouldn't have done shyt if he came out about the relationship with padme, they'd talk shyt but what were they really gonna do? He wouldn't have been the first grey jedi.

Instead he allowed the Sith to manipulate him into destroying the jedi and killing his wife.

Some of it was Anakin's fault, but a ton of blame was on the Jedi as well. Their doctrine forces them to never explore emotions, in that if you have extreme ones, it would lead you to the dark side.

The reason they wanted young kids, was so they can get them indoctrinated in that way of thinking, not because of skill level. They also relied on tradition and prophecy, instead of adjusting it to modern ways. Qui-Gon Jinn was the best teacher for Anakin, as he naturally rebelled against the teachings of the Jedi, as Obi-Wan in their short time on screen, always questioned his thinking.

The biggest mistake though was on Mace Windu. Anakin still believed in the Jedi order, but also accepted in teachings, 'Only a Sith deals in Absolutes'. Mace broke that rule in wanting to kill Palpatine. Although at the same time, Anakin should've stayed back as Mace would have killed him if Anakin didn't step in.

Even Yoda is to blame, as he always expressed doubt in Anakin instead of being supportive.

Maybe of he believed in himself like Bruce Leroy he too would have earned the rank of Master.
