It's exactly what they were talking about. Remember, Ellie got bit on purpose when the Rattlers found her strung up in the trap. After Ellie frees the prisoners and they say "she's bit!", ellie literally looks at the bite mark she just got lol.

He was talking about ellie being bit. She got bit again by that hanging clicker, she looks at the bite mark on her hand after dude tells her she can get to abby before the infection takes hold, not knowing she's immune.
Yeah but id assume you can still custom savePermadeath that carries across both characters?
No way they expect you to play a 25 hour game with no supplies, Smart AI & 1/2 hit kills with no Manuel saves for a bronze trophy lol
Edit: saying that, by the looks of it permadeath isn’t exclusive to Grounded so who knows

The only thing hard about the difficulty is the fact that they make your ally a complete dumbass. I had it on survivor but put ally ai on moderate and the experience was perfect, the AI did enough to not seem useless without feeling like they were carrying me the whole game.
Permadeath seems doable.
