In the old canon the sith were built just like the jedi but they lost because of in fighting so dude killed the rest and took an apprentice and started the rule of two. Eventually due to the accumulation of knowledge and the sith seeking strong apprentices who overtake and kill theory masters it would eventually lead to an unstoppable powerful sith (which palpatines are). If palpatine had succeeded in turning Rey or she didn't kill him he would have completed the goal.

Technically only the two from the direct lineage are considered real Sith. If there were lore they wouldn't be following the sith code anymore.

It’s both . Literally only 2 sith anywhere , and only a Master and Apprentice. In legends bane literally wiped out all the sith except for himself and his chosen apprentice . If any other sith popped up then that sect would consider them illegitimate, like when mayo and his brother cane back during the clone wars.
