The ritual of mitzitzo vfeh has been observed for the past 3,300 years. However, the only contemporary Jewish communities who continue its observance are the Chareidim (of all communities: Ashkenazi, Sefardi, Teimani), most notably Satmar and other Chassidishe groups. Although traditionally it is skin-to-skin contact, it isn't always necessarily performed that way. Even with a glass tube, it's still the פה (mouth) that's doing the מציצה (suction). Even so, most Orthodox Jews use a gauze pad or its equivalent and at no point during the procedure does the mohel put his mouth anywhere near the bris not even indirectly.
