Marx wrote that the reason people oppose communism is because they are so delusional that they either can't see, or refuse to see the reality that is staring back at them right before their eyes.
The delusion runs so deep in capitalist society. People don't live in the real world or see the reality. They live in a dream world instead and chase after fantasies, like a heroin addict chasing the dragon. That's what they sell you, the capitalist dream. The American dream, the Australian dream, whatever. Everybody believes that they are just on the cusp of becoming millionaires. They live as though they are temporarily embarrassed millionaires.
And when you live in a dream world like that, you can't deal with it when someone points out the reality to you - that under capitalism you will never be rich, you will never be a millionaire, you will never be powerful, you will always be in debt, you will always be working for somebody else creating products that they sell for profit, and for which they pay you only pennies on the dollar - you can't handle it. You would rather keep your dream alive and demonize the person who speaks truth to you, because the truth is so painful. Just see the replies in this thread. Not one word said about Marx's actul philosophy and his vision. Just deflections and name calling to demonize the person speaking the truth and to keep their fantasy intact.

Well maybe under a society where everyone is equal, with equal education since birth the truly crème of the crop would rise to the top and we would have even better surgeons
The retail worker and heart surgeon in your scenario could have very well switched positions in this life if they both had the others opportunities
