how is it more efficient than how Americans have it.. heating up those coils with electricity is prob very expensive in the US.
It isn't more efficient, the reason it's widely used it's because it's easier to install and maintain.

Biggest thing that surprised me is the amount of dust on top of it considering the water and likely steam. Bathrooms are usually the least dusty places.

The average house in many south- and centralamerican countries doesn't have running hot water. That's why they came up with these showerheads as an easy fix. In the DR, only the fancy luxury appartments in the high rises build for money laundering came with hot water. But that was in the early 2000's so things might have changed by now.
