Are you sure that no player had to switch numbers when the rule came into effect?

Grandfather clause. If you had the #42 at the time, you could keep wearing it until you changed numbers or stopped playing. Only those players though (so guys like Butch Huskey and Mo Vaughn could still wear 42 as well as Mo).
