I caught that too, recently. Interesting that the pentito said that it was Cosa Nostra that sits at the top of the supercupola, not the Ndrangheta or the Freemasons. I wonder if that's true or not.
It's hard to believe it at first since Cosa Nostra's waned and Ndrangheta's power has risen since the 90s. But Cosa Nostra was on top for half a century. They've been partners with the CIA and the Vatican in everything, Operation Gladio, the transatlantic heroin syndicate, washing money for bigshots all over the world. The elite members of Ndrangheta and Camorra were also inducted as Cosa Nostra members for further legitimacy and protection(like the Levante's in the show). Cosa Nostra also instructed Pablo Escobar's cartel on dealing with the state, and helped organize the various Russian families when the Soviet Union failed.That kind of power doesn't just go away.
As for the Freemasons, the popular conception is that the Giulio Andreotti types sit "above" the mafias, but the relationship is probably hardly that simple. Ever since Riina was arested there was talk of the blackmail files he had on Italy's elites that could bring the country down. The files supposedly passed to Provenzano and then to MMD. Well, it's been 25 years since then but many of those people(and their successors, family or otherwise) are still around.
It seems in recent years Cosa Nostra also made attempts at reunification and the state tried to nip it in the bud. Yearlier in the year Leandro Greco was arrested trying to reestablish the traditional cupola. It remains to be seen whether he succeeded or not.
