This gentleman has to immediately decrease calorie intake and lose a minimum of 75-100 pounds before he would even be discussed as a candidate for weight loss surgery, which at this point is probably what he needs. Otherwise he is a risk to die on the table.

He will have to eliminate fatty foods and mix in some sort of physical activity daily to help reach that weight loss goal. Once that happens, a gastric bypass would potentially be an option, but even then he would have to maintain a healthy lifestyle, on top of dealing with the excess skin that will be inevitable as a part of the weight loss.
