Side-effects:
Teleportation- Depending on where you teleport to, the momentum you experience at a different point can cause you to be knocked into a wall. Remember, the relative speed of the Earth's rotation is faster at the equator than it is at the poles. So you would only really be able to teleport about 100 miles per clip at best.
Time Travel- Your actions in the past can erase you from the future. Your actions in the future will make it impossible for you to return to your specific past, since time splits into multiple time streams.
Mind reading- You will be infected by their mental illnesses and their thoughts will germinate in your mind, no matter how obscene or insane, as your own.
Mind control- The side-effects of this one depends on what type.
Type A: If it is a type that you must actually be present to accomplish, then the side-effect is that the person knows you were the one who made them do it. Which could easily create a new nemesis who will seek to kill you, because you are an existential threat to their and anyone they love existence.
Type B: If it is the type that you can do from afar (i.e. possession like), then you will be blind, deaf, and lack any tactile senses to your original surroundings while mind controlling your victim. Essentially, your body will be in a coma while you are mind controlling a person. Leaving you vulnerable if you do not make sure you are in a very safe place. So not really that onerous of a side-effect in physical damage, but can have a very bad effect on your spiritual wellbeing and may also leave your body open to possession by another.
