If the Fullbacks push up like they've been doing. Insigne gonna torch that back line. The key been Sterling, Shaw and Phillips. Its not so much that the defense has been stellar for England...its the middle of the park. The back line isn't really having that much to do most times. Of the games I seen at least. I've grown VERY impressed with Phillips, Rice, Mount and Foden.
On the flipside, I'd start Sancho - Kane - Sterling and tell Sancho "cut in and have fun". Kane should just lean on the back shoulder and play physical with Chiellini. Raheem drop into the midfield and orchestrate. Mount or Foden come in as the late runner. Italy's back line is OLD AF. Play possession...don't let Italy counter and play a slow boring game.

The important players (KDB, Hazard, Lukaku, Courtois) will be at the WC, for sure.
That whole "He starts Sancho now that he plays for a Premier League team!" thing was just some social media nonsense.
But to answer the second question, that used to be the case back in the day due to not only the young English players having a comfortable life in England but also because foreign coaches believed the English players lacked the technical ability to compete.

Not many Italian players play outside of Italy either. The EPL is the top league in the world and Seria A although it has fallen quite a bit in recent years is still up there. So that's probably the main reason. For Italian players you could argue they're momma's boys
