I think its probably Witcher 3 for me, considering I’ve played it front to back twice and started a couple new games in it between then.

I know I put in a ton of time on both Horizons most recently. Almost at 40 hours on Forspoken right now too.

I haven't played that many open world games, so probably GTA3 or Vice City.
idk who's number 1 but my top 5 is easily Fallout 4, Skyrim, Oblivion, Fallout New Vegas, and Fallout 3. Played through all games multiple times and on 2-3 platforms each.
Honorable mention goes to Elden Ring at about 230 hours across my first and new game plus playthroughs. Witcher 3 is up there as well since I also played that game on PS4, Switch, and Series X. After that the gap is pretty large

Either Watchdogs 2, GTA V, or AC Origins.
It's gotta be either RDR or Fallout, I'm leaning toward it being Fallout 3 because I was playing that game like a complete puss at the time (couldn't handle horror well when I was younger), also neither benefits you much to fast travel in due to random occurances.
GTA and ES are long but I never spend a lot of time in the world, I go mission to mission pretty quickly.
Worst open worlds I've played that come to mind are probably LA Noire and BOTW despite both being actually good games. The open world in them feel more like an obstacle rather than a dynamic world.

Definitely, recently did a third completion, “deathwish” that shyt was hard af. Killed Michael in one and Trevor on another but hadn’t completed deathwish because that joint was so hard.
The Witcher 3. I put in 200 hours into the Witcher 3, but also 174 in Skyrim, and 120 in Fallout 4
