What happened to their face did they get the fat sucked out or something, they used to have such full faces even into their early 20s. Now one of them looks way beyond their years, they don’t even smile anymore it seems like.

Well over a decade of anorexia/bulimia, cocaine and heroin (or prescription versions of those drugs), smoking, drinking, uppers, no sleep, stress, and probably some kind of childhood trauma all lead to this. Plus, they're both White.
