I’m sure the mafia still around running small scams and doing legitimate business as well. But In America at least they have no real power. But I’m sure their still eating.
Last year a big New Jersey bust where they were making edibles in 2 warehouses got indicted and there was a few mob dudes involved in the fed sweep.

The Italian American Mafia never had just one boss or one family, nor did the ones anywhere else. The Italian American Mafia became semi-relevant after the 1990s and pretty much stopped being relevant after the 2000s for the most part.

The Italian Canadian Mafia and the Italian Australian Mafia are with the shyts though. The Canadian ones are probably a bigger player in the US then the American ones. They be exporting weed into the eastern US, and importing coke into Canada. They were exporting some heroin into the western US even in the 2010s but I'm not sure how much of that is still going on.
