It's not just about vaginal or even anal sex with women. There's the possibility that he was fukking with chicks who were doing hard drugs. There's also the possibility he was doing hard drugs.
Look at the HIV/AIDs rate in the black community right now - not all of that is due to DL dudes brehs. Having existing STDs increases your risk of contracting AIDs. That's really how that shyt spreads. IE someone gets gonorrhea, gets it cleared, then contracts it again but doesn't show symptoms and thus doesn't know he/she has it. And more unprotected sex just makes it worse until the breaking point - exposal to blood.
