I don't know, I just remember it being mentioned when highlights of Fallon's 1st night was shown.

Johnny Carson banned her because she took a job on FOX as host of The Late Show, which was direct competition to The Tonight Show. He was personally insulted, and he wanted her to stay as his backup whenever he didn't feel like workin.

After Johnny left, Jay Leno maintained that ban out of respect for Johnny.
