The phone thing is interesting, not to spoil it for anyone but if you know the origin of the phone you know Rick didn't realize what he did at the very beginning of the phone.

I really don't see this show doing on of those off screen kills. Lori's death was off screen but it was implied what was going to happen. Carol just ran off to a fate unknown I doubt they would just leave it that. But why dig the grave and then go to the trouble of "burying" an empty grave with no remains?
I know thye do that for people presumed dead like soldiers but they could have looked for Carol it's not like she could have gotten out of the prison, they knew where to search for her.
No its because it could be a possible spoiler.
I also just thought about something what if ....
That whole scene with Rick finding that bullet in tact and that zombie is buggin me. Something just ain't right.
what if Lori and Carol arent dead and Carol came back and saved Lori somehow and stitched her up? Remember that scene where she cut that walkers stomach open outside?
good point...that would be a great spin on things compared to the comic.

Well on talking dead they said the zombie ate lori and you can see her hair on its face and hands . Plus the bullet is from Carl shooting it would take a lot of stitching to save her
