Someone else mentioned this earlier, but I think Naruto will "die" and Sasuke will become Hokage. Nartuo's death has been forshadowed throughout the manga; IE beginning of the manga when Naruto says he will be on the Memorial stone even though he didn't know it was for dead ninjas, the end of the Valley of the End fight with Sasuke looking down on Naruto as the light seems to fade away from Naruto and Kakashi seeing Naruto's fallen body and remarking that this may have been Naruto's fate all along, the Kyuubi cryptically telling Sasuke that he'll regret killing Naruto, probably some more I'm forgetting.
Naruto's main reasons for wanting to become Hokage was to be acknowledged (which he's gotten ages ago) and achieving world peace, in a nutshell. With the peace part, Naruto doesn't literally have to be Hokage to achieve that. Now we've got Itachi's words that one who becomes Hokage doesn't necessarily have to be acknowledged as the Hokage. I think that Naruto will eventually become the personification of the Will of Fire, with Sasuke becoming Hokage and carrying on Naruto's will as part of his redemption.
Sounds a little dumb typing all this out, but I feel it's a very likely scenario.
