You can do multiplayer with everyone, but it will only show you the 50 closest people, multiplayer with only your friends, or nobody at all. Additionally you can do live traffic (like real world planes and their real time paths [go here on your computer FlightAware - Flight Tracker / Flight Status]), AI generated traffic, or no traffic at all.

You should hold off until they say for certain that a flight stick can be used with the Xbox. I don't think that's confirmed.
