Carol probably didn't realize just how emotionally disturbed Lizzie was. Clearly Lizzie had problems going to back to before the outbreak, Carol probably only thought it only started during the outbreak or was because of it thinking she would adjust.

That "look at the flowers, Lizzie" thing seems like that was a routine she and Mika (RIP) did way before the outbreak.
