You have to clear the chamber two to three times to make sure its clear. I do this for all of my guns before I clean them.

Sometimes if you don't pull the slide all the way, it will remain in the chamber.

As an experienced gun owner I would like to point out that the fact that there was no clip in the gun means the bullet was already in the chamber. She only pulled back the slide a quarter of the way which is the reason the round wasn't expelled and was still loaded.
