If that relationship doesn't work out, it's going to be awkward for everyone involved.
You, the ex and the mutual friend.

That mutual friend has to either pick a side, which is going to hurt one person.
Or carefully act like a middle man with no opinion, which is just going to make them come across as fake.

Just one big mess, the way I see it.

It happened once for me...but my friend who hooked me up with this girl I smashed did so cause she wanted nothing to do with him and he tried to act like he was too good for them. And then he tried to take credit for it when really she wasn't feeling him at all. So it was bound to happen.
