The whole movie was a dream, hence a person going to sleep in a dingy apartment before the opening dance sequence. All of the people in the dream played a significant part in Naomi Watts character's real life, but in far different, more fukked up ways. When she wakes up it ties together all those elements of her dream and their real life implications.

Twist is something that happens at the end of a movie which changes your perception of the story.

Empire Strikes Back was the second part of a trilogy so it wasnt exactly the end of the story and although Vader being Luke's was a good shock, it wasnt that significant to the overall narrative.

Dearth Vadar being Luke's father was actually very significant to the narrative. It changed the dynamics completely
