How do you not lock out/tag out your machine before performing maintenance. If there were no procedures the whole plant should be shut down, either way, MexiMelt should have known better. I remember something similar happening at the Tyson chicken plant years back when a man fell into a vat trying to clean it. Because the company had procedures and he didn't follow them, he was technically fired moments before he died :manny:
