To add to this, promethazine is just an antihistamine. Think Benadryl…stops allergic symptoms like runny nose, will make you drowsy, etc.

The star of the show is the codeine, an opioid. Codeine gets converted directly to morphine by your liver. It’s supposed to be a powerful cough suppressant but it’s not that great tbh for most people.
