It isn't that hard. I'm not even black or American/Caribbean, and I can understand the lyrics. If you listen to dancehall music enough you can sort of get the basic gist. And if you look up lyrics, Patois is much easier to understand by reading instead of listening.
I guess people just too lazy to go to the trouble of having to learn a different language just to listen to music
