There is cross-sectional evidence that impairment of thyroid function evidenced in mothers and neonates in conditions of mild-to-moderate iodine deficiency affects the intellectual development of their offspring. Aghini-Lombardi et al. (44) reported that in children aged 6-10 years in an area in Tuscany who had mild iodine deficiency (64 μg iodine/day), the reaction time was delayed compared with matched controls from an iodine sufficient area (142 μg iodine/day). The cognitive abilities of the children were not affected. Similarly, it was reported that in an area of Southern Spain with mild iodine deficiency (median urinary iodine of 90 μg/L), the intelligence quotient (IQ) was significantly higher in children with urinary iodine levels above 100 μg/L (45). In severe iodine deficiency, the frequency distribution of IQ in normal appearing children is shifted towards low values as compared to children who were not exposed to in utero iodine deficiency because of correction of the deficiency in the mothers before or during early gestation (48-49). In a meta-analysis of 19 studies on neuromotor and cognitive functions in conditions of moderate to severe iodine deficiency, Bleichrodt and Born (50) concluded that iodine deficiency resulted in a loss of 13.5 IQ points at the level of the global population. A more recent metanalysis conducted on studies in China produced a very similar result (51).
The term IDD refers to all the ill-effects of iodine deficiency in a population that can be prevented by ensuring that the population has an adequate intake of iodine (1). These effects are listed in Table 2. Brain damage and irreversible mental retardation are the most important disorders induced by iodine deficiency: in 1990 it was estimated that among the 1572 million people in the world exposed to iodine deficiency (28.9 % of the then world population), 11.2 million were affected by overt cretinism, the most extreme form of mental retardation due to iodine deficiency and that another 43 million people were affected by some degree of intellectual impairment (18). Thus, iodine deficiency was a leading global cause of preventable mental impairment.
This study quantifies the effects of iodine on the intellectual development of children using a systematic manual literature search of Chinese publications related to iodine deficiency disorders. The Chinese Medical Reference Database, Medline, and Cochrane library were searched electronically in Chinese and English. Inclusion criteria included: studies conducted in China, comparing children (<16 ys) living in naturally iodine sufficient (IS) with those in severely iodine deficient (ID) areas, or children in ID areas born before and after the introduction of iodine supplementation. Intelligent Quotient (IQ) was measured using Binet or Raven Scales. The iodine sufficient control groups were comparable socially, economically, and educationally with the study groups. Random effects models were used in the meta-analysis. Effect size was the standard deviation IQ point (SIQP), which is equivalent to 15 IQ. Thirty-seven reported studies, total 12,291 children, were analysed. The effect size was an increase of 0.83, 0.82, and 0.32 SIQP respectively, for the children living in IS communities compared with those living in ID areas with no iodine supplementation, with inadequate iodine supplementation, or children who had received iodine during their mothers’ pregnancy and after birth. These equal to 12.45, 12.3, 4.8 IQ points. Compared with that of children whose mothers were persistently exposed to ID, the total effect size of the 21 entries was an increase of 0.58 SIQP (8.7 IQ points) in the group receiving iodine supplementation during pregnancy. Furthermore, there was an increase on 1.15 SIQP of Binet or 0.8 SIQP on Raven Scale (17.25 or 12 IQ points) for children born more than 3.5 years after iodine supplementation program was introduced. The level of iodine nutrition plays a crucial role in the intellectual development of children. The intelligence damage of children exposed to severe ID was profound, demonstrated by 12.45 IQ points loss and they recovered 8.7 IQ points with iodine supplementation or IS before and during pregnancy. Iodine supplementation before and during pregnancy to women living in severe ID areas could prevent their children from intelligence deficit. This effect becomes evident in children born 3.5 years after the iodine supplementation program was introduced.
The association between iodine deficiency and poor mental and psychomotor development is known. However, most studies were undertaken in areas of very low iodine intake. We investigated whether a similar association is found in schoolchildren from southern Europe with a median urinary iodine output of 90 μg/liter. Urinary iodine levels were measured in 1221 children who also completed a questionnaire about their usual dietary habits. Intelligence quotient (IQ) was measured by Cattell’s g factor test. IQ was significantly higher in children with urinary iodine levels above 100 μg/liter. The risk of having an IQ below the 25th percentile was significantly related to the intake of noniodized salt and drinking milk less than once a day. As expected, the risk of having an IQ below 70 was greater in children with urinary iodine levels less than 100 μg/liter. In conclusion, this study demonstrates that the IQ of schoolchildren in a developed country can be influenced by iodine intake. The results support the possibility of improving the IQ of many children from areas with mild iodine deficiency by ensuring an iodine intake sufficient to achieve a urinary iodine concentration greater than 100 μg/liter.
