so far no dull moments climbing the ladder for the titles. Always found those cringe in some ladder matches when the person is climbing so slow to aid a certain spot if it's somewhat late.

I think it makes sense in kayfabe terms too. Ref isn't trying to see anyone die

I remember the refs doing it as far as back Cena vs Edge in 06 in the TLC match

I think it started back in 2000s when Jeff Hardy started using those really tall ladders. They just did a great job of camera work to not expose the refs, but once in awhile you can spot them.
