To be fair, they didn't make it 100% obvious. But you could see when they tested him, the screen came back red, so he was done for. I just put two and two together when I saw Joel dump a kid's body on the pyre.

You're supposed to realize what's happening when they starting promising her unrealistic gifts of toys (in a world with limited resources) as they're injecting her with a needle full of an unknown substance

And they injected the kid right through their shirt which means it wasn’t really about making them better.

Also remember a pretty violent scene in like an apartment complex where Joel kills a bunch of people and even throws someone through a wall or something? Can't wait to see that on the show.
