It was a lot of things that did him in, but Benny pulled that trigger.
The biggest lesson of all this is a favor will kill you quicker than a bullet. All them favors for people put him in dangerous situations and eventually got him killed.

there are basically two things that did him in:
a) times had changed and he was unwilling or incapable to adapt
b) misplaced loyalty
the lesson of the movie is that sooner or later, life on the street will catch up with you. sure, benny pulled the trigger, but he was only one of many factors that were about to catch up to carlito. he had the cops working hard to put him back in jail, his lawyer selling him out, his close associates looking at him as weak, the italians trying to find him and benny blanco plotting for revenge. one of these things would have did him in anyway, it's not important which one, beause he was already on a path to self-destruction the moment he chose life on the streets.
