I'm not even trying to be that dude or anything but just didn't even find it funny.
Either he had some trauma such as burns or trauma to his scalp that prevents hair from growing back. Or he may have have some issues that arise in children specifically that cause hair loss. The fact that they are round, oval patches, and smooth points to some child hood alopecia issues or a ringworm infection.

Ringworm in children and hairloss. Most likely cause. This can only gotten rid of by anti-fungal treatment and given its a pretty impoverished area, this may be a sign that he simply doesn't have the healthcare needed to treat the issue which is a whole other issue in itself.
